Camden Council’s ASC Neighbourhoods Service is the largest service in ASC

ASC Neighbourhoods Service

The ASC Neighbourhoods Service supports people from age 18 years with physical and/or sensory needs and older adults who have care and support needs. We promote their independence and well-being to enable them to live healthy and independent lives within their communities.

The ASC Neighbourhoods Service comprises the following colleagues:

  • Service Managers are the strategic leads for our Service.
  • Team Managers manage our Neighbourhood and Hospital Teams.
  • Social Workers and Social Care Practitioners work with individuals and their network to understand what support a person may need to live an independent life and ensure that individuals are provided with the information that they need in order to make informed decisions.
  • Occupational Therapists work with individuals and their network to understand the equipment and/or adaptation and telecare options that can support the person to remain independent in their home.
  • Outreach Officers work directly with individuals in collaboration with our wider social care teams to ensure that people are supported to maintain a safe home environment.
  • Rehabilitation Officers support people who are blind or partially sighted, and/or Deaf or Hard-of-Hearing to live independent lives.
  • Team Clerks provide administrative and business support to ensure that our social care teams run smoothly.
