Care reviews

“A review should be proportionate to the circumstances of the adult”.

Reviews should focus on the personal outcomes of the adult, the support plan, use of the personal budget and should ensure that the service provision continues to be tailored to meet the needs and personal outcomes of the adult.

Placement Reviews guidance is available for carrying out reviews with Camden residents who have moved to a care setting either within or out of the borough.

The results of the review may identify potential or imminent risks to the adult or others, therefore any risks identified must be addressed by the allocated social worker. Proportionate and effective risk planning with the adult and relevant agencies should be completed to help minimise the identified risks.

For any safeguarding concerns identified during the review the allocated social worker or social care practitioner should follow Camden Councils local appendices to the London Safeguarding Adult’s Policy and Practice Guidance.

If during or following a review it is identified that the adults direct payment is being misused (whether intentionally or because of potential abuse) the allocated social worker or social care practitioner should refer to the Direct Payment Policy and Practice Guidance.

All discussions and information obtained during the review and regarding risk or safeguarding must be recorded accurately.
