Care reviews

“A review should be proportionate to the circumstances of the adult”.

Reviews should focus on the personal outcomes of the adult, the support plan, use of the personal budget and should ensure that the service provision continues to be tailored to meet the needs and personal outcomes of the adult.

Placement Reviews guidance is available for carrying out reviews with Camden residents who have moved to a care setting either within or out of the borough.

Reviews should not be used as a tool or resource to amend personal budgets to save funds. However the information collected in the review will determine whether the eligibility criteria should be reapplied and updated on mosaic.

Any changes identified that would have an impact on eligible needs should be discussed with the adult and clearly recorded on mosaic.

The review should consider:

  • the support provided by the adult’s personal network
  • whether a carer’s assessment should be offered
  • if voluntary organisations could be considered to offer support
  •  if equipment and/or adaptations can or have been provided to support the adults needs,
    reduce risk and increase independence
  • whether the adult would benefit from reablement
