Care reviews

“A review should be proportionate to the circumstances of the adult”.

Reviews should focus on the personal outcomes of the adult, the support plan, use of the personal budget and should ensure that the service provision continues to be tailored to meet the needs and personal outcomes of the adult.

Placement Reviews guidance is available for carrying out reviews with Camden residents who have moved to a care setting either within or out of the borough.

A review should be completed by appropriately qualified staff within 12 weeks of the commencement of any new service or support or 4-6 weeks where possible. Following this the review should take place annually or sooner if required. Reviews can be requested at any time by the adult, carer or family friend however the allocated social worker or social care practitioner is expected to consider the appropriateness of completing a requested review.

The review date should be agreed between the social worker, social care practitioner and the adult and confirmed during the initial assessment stages. The type of services the adult is receiving, their support network and the needs of the carer if applicable should also be taken into consideration, including whether other professional’s or service providers should be invited to participate in the review. However if other professionals are invited to the review this should be discussed and agreed with the adult first.

The adult can choose how and where their review takes place, however the allocated social worker or social care practitioner should also ensure that the most suitable communication method is used during the review (telephone or face to face, use of communication aids, interpreter) which should be discussed and agreed with the adult.
