Guidance and best practice on care reviews

Care reviews

“A review should be proportionate to the circumstances of the adult”.

Reviews should focus on the personal outcomes of the adult, the support plan, use of the personal budget and should ensure that the service provision continues to be tailored to meet the needs and personal outcomes of the adult.

Placement Reviews guidance is available for carrying out reviews with Camden residents who have moved to a care setting either within or out of the borough.

Where a change in need is identified as a result of a review the allocated social worker or social care practitioner should:

  • complete a reassessment that is proportionate to the change
  • clearly document the reasons for any increases or decrease in levels of support
  • recalculate the personal budget
  • update the support plan and mosaic
  • ensure that the review document and updated support plan is signed by both the adult and allocated practitioner

For adults who no longer have eligible needs they should be signposted to other agencies and provided with information, advice and guidance on how to access local and community based services.

Information on how to get back in touch with Adult Social Care if the adults care and support needs increase should also be provided. If the adult does not agree with the changes made further to the review the allocated social worker should seek to find a resolution with the adult. If a resolution cannot be reached the adult should be provided with the adult social care complaints procedure.
