What Matters: Three Conversations

What Matters is proudly Camden’s approach to delivering Adult Social Care (ASC) support since 2019.  It is our attempt at placing people at the heart of everything that we do, and it recognises that people are the experts in their own lives.  Through the approach, we support people, carers, families, and communities to focus on what really matters to them and collectively, we find the best solutions to the difficulties they face.

Supported Self Assessments – Practice guidance

Why do we offer supported self assessments?

Under the Care Act, a local authority should offer an individual the opportunity to undertake a supported self-assessment which places the individual fully in control of the assessment.  While very few people request a self assessment, if an individual wishes to do a self-assessment, the local authority has to provide the necessary information, advice, and support.

The supported self-assessment should use the same assessment materials as used in other assessments of needs and the local authority must assure itself that it is an accurate reflection of the individual’s needs.  A Camden self assessment form has been developed which is more accessible version than the tools and systems used by practitioners to have a conversation with a person for assessment.

There are currently two versions available of the self assessment form.  They are available on request only (they are not available on the Camden website):

The form can be completed by the person, or on behalf of the person.  The form asks the person completing the form to indicate if they are completing it on someone else’s behalf.

Process for self assessments

Forms are available on request.  They can be emailed or posted to the person, according to their preference.  Forms can be completed online or by hand, and the person can ask someone else to help them with the form, including a social care practitioner.  It can also be completed on behalf of the person, should they choose.

The completed form should then be sent back by email to [email protected] or by post to the Town Hall, which will be opened by the Team Clerks.

Should someone request a self assessment, completion and return of the form is the first step in the assessment process.   Once the form is received, it should be forwarded to the allocated worker, or to Duty to pick up.  The relevant officer should be scanned (if a hard copy) and attached to the person’s main file on MOSAIC, under ‘Documents’.  They should then review the person’s form and contact them to arrange a follow up conversation, and to complete Conversations as necessary to determine the support the person needs.
