What Matters: Three Conversations

What Matters is proudly Camden’s approach to delivering Adult Social Care (ASC) support since 2019.  It is our attempt at placing people at the heart of everything that we do, and it recognises that people are the experts in their own lives.  Through the approach, we support people, carers, families, and communities to focus on what really matters to them and collectively, we find the best solutions to the difficulties they face.

Some care providers are changing from hand written notes to digital care plans. This new system will make sure residents are aware of the care they need and make access to notes easier with a quicker process.

 

Residents can download an application on their phone or other device. The care provider will provide them with the password.

 

The person's family and other people involved, with permission from the individual, can have access to the digital care plan to know what support they are getting day to day. Further details on digital care plans can be found in the access to digital care plans notes for residents.
