Reablement

Reablement is a service that can support people to improve, develop and regain the confidence and skills needed to manage activities of daily living and live as independently as possible.  Reablement support usually takes place in the person’s home, but where that is not possible, Camden has a small number of reablement flats at  which can be used for short-term reablement.

There are 13 adapted reablement flats at Henderson Court, available where reablement support cannot be delivered within a person's own home.  Attached is information about Henderson Court flats including guidance on how to refer a person - Reablement flats at Henderson Court Guidance.

Who is Reablement for

The service is for adults who live in Camden who have experienced a short-term crisis because of their situation such as an illness, an injury or an accident.

Where can it be provided

Reablement can take place either in a person’s home or at Henderson Court which is a small number of reablement flats at Henderson Court sheltered housing scheme in Hampstead.  

People who could benefit from a period of reablement, but whose home is not suitable can be referred to Henderson Court .

Reablement at home will be based on a Reablement Support Plan completed by the adult social care practitioner. The goals that an individual wants to achieve through their reablement will be agreed jointly with the person.

Reablement goals will be reviewed regularly to ensure that they are relevant and achievable.
