Information about Camden Mental Health Services and the practitioners that deliver the range of care and support that is available.

C&I Mental Health Trust

Mental Health Services in Camden are provided in partnership with the Camden & Islington Foundation Trust (C&I) and Camden Council as part of the a Section 75 agreement.  This section provides an overview about the range of integrated health and social care services that are available and social care leads that deliver specialist care and support to people in Camden with mental ill health.

Mental Health assessments pathway

  • The pathway applies to any adult whose primary need relates to Mental Health.
  • Mental Health social care activity is carried out within Integrated Health and Care Teams (in the Camden & Islington Mental Health Trust).
  • Health and/or social care activity might include carrying out social care assessments and reviews alongside/integrated with health activity.
  • Health activity may lead on some interventions, e.g. crisis.

Channels for responding to referrals for MH assessments

There are two channels:

Secondary MH care service – responds to people that are known to the service or meet secondary specialist criteria for support

Primary Core service – responds to people that are not known to secondary MH care service or do not meet criteria

ASC involvement

  • ASC do not have involvement in the crisis stage. Involvement happens in the primary or secondary space. If the person is in secondary care, they will have a Care Coordinator who will identity their social care needs. Any social care needs identified goes back to the team that is responsible for the person Eg. if person is under care of the personality disorder team, then that team is responsible for picking up their social care needs.
  • Once the person’s MH needs are stabilised the Conversations are practised or completed as follows:

            - Conversation 1 and 2 – A MH practitioner in relevant Multidisciplinary Team (MDT) practises Conversations 1 and 2.

            - Conversation 3 - is recorded on MOSAIC. If long term care and support is required, a decision to commission this goes to Mental Health Quality Assurance Panel (QAP) for approval.

  • The primary recording system for the MDTs is RIO. Key social care activity data is inputted into MOSAIC at key points illustrated in the process map.
