Best practice to ensure that patients are safely discharged from hospital

Hospital discharge

Hospital discharge refers to when a patient is ready to leave hospital and return to their normal place of residence. This section provides best practice to ensure a person is discharged safely, including: discharge pathway, reablement flats, safeguarding raised in hospital, discharge alert protocol, and support for carers that are identified as part of the discharge process. 

If someone in hospital has an informal/unpaid carer supporting them, it is best practice to include the carer in the discharge process.

This is also an opportunity to identify carers who are not receiving support that they may need.

There is currently Hospital Discharge Support available for carers through Camden Carers Service.  The carer or person being cared for must live in Camden.

Currently the hospital discharge support includes:

  1. Priority registration and access to services
  2. Priority Carer Conversation/Review
  3. Access to the Hospital Discharge Fund

Hospital Discharge Fund

This aims to support hospital discharge, the sum is usually anywhere between £100-£300, in the past we have funded:

  1. Food
  2. Energy bills
  3. Travel to/from hospital
  4. Respite for carer

Camden Carers look at each referral on a case by case basis and if for example, additional care for person being cared for is needed, the amount awarded may be higher. The team are happy to fill in the grant referral form with carers upon referral.

How to refer

Please use this form to refer a carer to Camden Carers for priority registration and access to services, or for a priority Carer Conversation/Review Camden.

Camden Carers don’t want the referral forms to put workers off making referrals if they are too time consuming. They are happy to accept referrals which include basic details including, carer name, contact details and brief description of the need/situation – as long as the carer has given consent (verbal is fine) for the referral
