Reablement

Reablement is a service that can support people to improve, develop and regain the confidence and skills needed to manage activities of daily living and live as independently as possible.  Reablement support usually takes place in the person’s home, but where that is not possible, Camden has a small number of reablement flats at  which can be used for short-term reablement.

There are 13 adapted reablement flats at Henderson Court, available where reablement support cannot be delivered within a person's own home.  Attached is information about Henderson Court flats including guidance on how to refer a person - Reablement flats at Henderson Court Guidance.

Moving from Reablement to Ongoing Care

When reablement ends, practitioners must ensure there is a smooth, safe handover to any new home care provider.

What you need to do

  • Complete the final reablement review and confirm whether ongoing care is needed.
  • If ongoing care is required, finalise the support plan in Mosaic and include arrangements for an in‑person or virtual handover between providers.
  • Send the support plan to RCT at least three working days before ongoing care is due to start.
  • Keep the person informed about the new provider and handover arrangements.

 

What happens next

  • RCT will source a provider, confirm the start date, and manage any delays.
  • Providers arrange the handover on or before the last day of reablement.
  • Practitioners should follow up within 24 hours to check the new support has begun and everything is working well.
