Mental Health Teams

This section provides an overview of the multi-disciplinary teams within Mental Health. 

Mental Health Teams

From 30th June 2025, a new model of Mental Health Social Work has been introduced in Camden.

Mental Health Social Work Teams

There are three Locality Mental Health Social Work Teams: North, East, and South covering different geographical areas of Camden. These teams work closely with NLFT’s Community Mental Health Teams and support residents with ongoing or complex social care needs. 

Each team: 

  • Carries out Care Act assessments, support planning, and reviews. 

  •  Completes Section 42(1) and Section 42(2) enquiries for people already open to the team. 

  • Completes Section 42(2) enquiries for people not open to ASC when allocated by MASH or the Hub. 

  • Works with NLFT in joint care planning and risk management. 

  • Provides short and long-term support to promote independence. 

  • Offers a daily duty system managed by a Senior Practitioner. 

 

Routes Into the Locality Teams: 

Locality Teams receive referrals for adults already open to the team from: 

  • The Hub 

  • NLFT Mental Health Multi-Disciplinary Teams 

  • Internal ASC teams (Neighbourhood Teams, MASH 

  • Internal non-ASC services (Housing, Welfare Rights) 

  • External partners (GPs, voluntary and community organisations) 

 

Referral routes include: 

  • Team email inbox 

  • Direct phone line (urgent) 

  • Mosaic workflow 

 

Duty Function Within Locality Teams: 

Each locality team operates its own duty function responsible for managing urgent or unplanned work, including: 

  • New work triaged by the Hub, MASH, or mental health Multi-Disciplinary Teams as urgent. 

  • Unplanned work arising from the team's existing caseload. 

  • Situations where an allocated worker is unavailable. 

Duty is overseen by the Senior Practitioner, and Team Managers determine how duty is organised.
