This section provides an overview of the multi-disciplinary teams within Mental Health. 

Mental Health Teams

From 30th June 2025, a new model of Mental Health Social Work has been introduced in Camden.

Mental Health Social Work Hub (Front Door)

The Hub is the main entry point for all new mental health related social care referrals. It is the first point of contact for adults whose needs may be connected to their mental health, ensuring that every enquiry is screened, risk-assessed, and directed appropriately. The Hub receives referrals from; 

  • Members of the public (via Contact Camden) 

  • Internal ASC teams (Neighbourhood Teams, MASH) 

  • Internal non-ASC teams (Housing, Welfare Rights) 

  • External partners (GPs, voluntary and community sector) 

 

There are three routes into the Hub:

The route used depends on urgency and the source of the referral. 

 

The Hub’s role includes: 

  • Receiving and triaging all new referrals for adults not already open to a Mental Health Social Work Team or NLFT. 

  • Providing information, advice, and guidance to residents, carers, and professionals. 

  • Screening for risk and urgency, including self-neglect, crisis, or safeguarding concerns. 

  • Allocating cases to the most appropriate team for assessment or ongoing work. 

  • Linking people with community or voluntary sector services where formal ASC involvement is not required.
