Hospital Discharge Service
The Hospital Discharge Function ensures that residents admitted to psychiatric hospitals receive timely, well-planned support from Adult Social Care.
A Senior Practitioner from Adult Social Care is embedded within NLFT’s Discharge Hub, working across Highgate Mental Health Centre, St Pancras Hospital wards, and the Ruby Ward (Royal Free Hospital).
Key responsibilities:

Coordinating discharge planning for people with social care needs.

Completing or overseeing Care Act assessments and arranging community support.

Liaising with ward staff, families, and community teams.

Ensuring Section 117 aftercare is in place where applicable.

Supporting Camden residents returning from out of borough placements.
