Making referrals

This section will cover how to make referrals to CLDS, how decisions are made, and referral criteria that must be met to make appropriate referrals. 

We have a referral form that can be completed to make new referrals for people who are not already known to us. Please complete the referral form and send it to [email protected] or [email protected].

Please note a person must have a GP in Camden in order to be considered for health input and must live in Camden to be considered for social care input from CLDS. If they live in Camden but have a GP in a different borough, please refer to that borough’s learning disability service to determine their eligibility for learning disabilities services. Any identified social care needs will be the responsibility of the London Borough of Camden.

New referrals are initially screened by our management team using information included in the referral form and other information supplied or available to us through Council and Camden and Islington NHS recording systems. The focus of initial screening is usually on the referred person's developmental history, educational achievements and adaptive functioning in relation to global learning disability range. Referrals are then responded to at this stage with a decision about eligibility for CLDS input.
