CLDS
The project has established a single point of access for all services and opportunities/activities for people with learning disabilities. The purpose of a single point of access is to coordinate better across all learning disability and mainstream providers to ensure that people have self-determination and participate in a broad range of opportunities and activities where they can learn and grow as individuals.
The single point of access takes the form of monthly Opportunities Planning Groups (OPGs) where a Practitioner and opportunities providers (Disability Job Hub, Centre 404, Unity Works, and others) sit down together and discuss options that are available to the person and agree a plan in line with their About Me information. The person and their carer are also invited to attend the group.
The OPG keeps an updated directory of opportunities/activities that is regularly updated and shared with CLDS
Special OPGs are available for College leavers. These are held early in the academic year to ensure there is a clear plan for when the person leaves college.
How to refer to the Single Point of Access Opportunities Planning Group
Please contact Louise Roberts in the Commissioning Team. Louise will then send you a slot time on the OPG, a Terms of Reference, and a one-page profile to complete about the person being referred.
