Camden’s ASC Neighbourhoods Service supports people from aged 18, including autistic adults, with a range of needs that relate to physical or mental health, frailty, homelessness or drug and alcohol dependence. We promote their independence and well-being to enable them to live healthy and independent lives within their communities.

ASC Neighbourhood Quality Assurance Panel

This document sets out the standard operating principles for the Neighbourhoods Quality Assurance panel. This procedure relates to all staff within Adult Social Care (ASC) that have a responsibility for Case Management, including Sensory Needs, Safeguarding, Mental Health and are members of the Quality Assurance panel.

The Adult Social Care service is for people, aged 18+ who reside in the community or hospital and have been identified as having care and support needs. The aim of this document is to set out guidelines for the evaluation and funding of all services. This is with the objective of enabling equity in decision making and embedding consistent, excellent assessment practice. The aim is to promote strengths-based outcomes that achieve value for money.

The Standard operating principles apply to all new or requests for increased expenditure within Adult Social Care including funding support for people leaving Hospital. The exception being that QAP review for hospital discharges, need to take place retrospectively.

How to join the ASC Neighbourhood Quality Assurance Panel

Instructions for adding cases and joining the ASC Neighbourhood Quality Assurance Panel (QAP)

Following a review of the proposed case by Team Managers, all QAP cases should be added to the shared calendar, within their allocated time slots. All Team Managers should have been given access the shared calendar NH QAP (please email: [email protected] or [email protected] if you  do not have access). Within the calendar there will be seven  appointment slots for each team every Thursday from 1pm until 5pm. Ideally, we need to have the calendar requests completed by 5pm on Wednesday, where possible.

The QAP Rota

The schedule for presenting cases will be embedded within a shared MS Outlook calendar which will rotate across 5 different timeslots between 13:30pm and 16:10 for neighbourhoods, each lasting 40 minutes. Hospital discharges will be at the same time each week (13:00 for 30 mins) and Specialist Support Team (16:50 for 10 mins). Reablement, Placement reviews & Safeguarding will join according to the neighbourhood associated with the resident. Time slots for the five neighbourhood teams will vary each week, allowing every team to join at different times between 13:30 and 16:50. This will be reflected in the Shared calendar which will have a 5-week rota cycle.

Adding Cases to QAP

To add a case, include the name of the practitioner that will be presenting the case in the first column of the table. In the next column, add the name of the resident and MOSAIC number (in the format outlined) in columns 1 and 2. The process for Hospital Discharges and SST will be the same. The placement reviews team will add their cases based on where the resident is situated geographically within the neighbourhood team. All team leads should have access to the shared calendar, including Specialist Support, Placement Reviews and Reablement Teams. These Teams may want to double check with their respective neighbourhood Team Lead to negotiate timings for presenting within their allocated timeslot. If a Neighbourhood has no cases to present it may be possible for another team to present earlier or later than their allocated slot. The link to join QAP will be the same each week.

Joining the QAP panel

A room has been booked for those that are able to attend the panel in person. This will be added to the QAP calendar each week for reference.  For those than cannot attend in person, a MS Teams link has been added to the calendar invite. The team manager will need to share the link to join QAP with the practitioner if they do not have access to the shared calendar. Practitioners will not be expected to stay for the entire duration of the QAP panel, only the time that it takes to present their case.

Tracking cases

All information regarding cases presented to the Quality Assurance Panel, should be tracked via MOSAIC with relevant details to enable a seamless process from request to requisition of care package.
