What Matters Workflow Refresh – Training Overview
As part of the What Matters workflow refresh, a series of training sessions were delivered tailored to the different teams within Adult Social Care. Below is an overview of each training group, along with a link to the slides used during the sessions.
This training is designed for practitioners and managers working in our operational teams.
It covers the full What Matters workflow and supports consistent practice across services.
Teams included:
- Neighbourhood Teams
- Specialist Support Team
- Learning Disability Services
- Children and Young People with Disabilities Service
- Mental Health Services
Session 2 – Hospital Discharge Teams
This training is designed for those practitioners and managers who work in our Hospital Discharge Teams and who support discharges from acute hospital settings via the Discharge to Assess (D2A) process.
It focuses on the workflow steps and recording requirements specific to hospital discharge pathways.
Please note:
This session does not apply to staff working with discharges from mental health hospitals or settings, who should instead follow operational team guidance.
This is a high‑level overview of the refreshed workflow and the key changes made.
It is intended mainly for non-operational staff and managers, or operational colleagues who do not typically record within the What Matters workflow.
This session is ideal for:
- Data Analysts
- Resource Coordination Team
- Other supporting or corporate teams
