What Matters: our approach in Camden

What Matters is proudly Camden’s approach to delivering Adult Social Care (ASC) support since 2019.  It is our attempt at placing people at the heart of everything that we do, and it recognises that people are the experts in their own lives.  Through the approach, we support people, carers, families, and communities to focus on what really matters to them and collectively, we find the best solutions to the difficulties they face.

Conversation 2 – Working intensively with people in crisis

Supporting someone in crisis to regain stability and control over their life.  Recognising that crisis means different things for different people.

Conversation 2 IS about:

  • Understanding what is causing the crisis
  • Working with the person and their network to put together an ‘emergency plan’
  • Managing risks positively
  • Sticking with the person to ensure changes are implemented and that plan works for them
  • Having small amounts of money easily available to deal with crisis
  • Supporting the person as to how they need to be supported

Conversation 2 is NOT about:

  • Determining Care Act eligibility
  • Long term support
