Mosaic

Mosaic is the main database for Adult Social Care and for Childrens services. It's a records and finance management system.

UPDATE: New Deep Clean & Decluttering Referral Process (Went live on 27th Feb 2026)

 

WHY THIS CHANGE?

  • To support the launch of the new Deep Clean & Decluttering service delivered by ClearUp Solutions.
  • To ensure referrals are consistent, complete, and easier for practitioners to progress within Mosaic.
  • To improve quality of information shared with the provider, helping them quote accurately and respond quickly.

THE CHANGES

1. New Deep Clean Referral Workflow Step in Mosaic

·       A dedicated Deep Clean Referral workflow step is now available from multiple What Matters and Safeguarding pathways.

·       You can add it as a Next Action, including the option to “Send immediately”.

 

 2. New Referral Form within the workflow step

·       Three sections covering: Guidance, Referral details, Management authorisation

 

3. New Requirement: PDF Output for Provider

·       Section 2 must be output as a PDF and emailed to ClearUp Solutions as the formal referral.

·       Quotation from provider must then be uploaded back into the referral step.

 

4. Management Authorisation

·       Quotation must be authorised by a manager using the Management Authorisation request.

·       Clear and structured decision fields added for managers.

 

 5. New Next Action Options

·       Purchase Deep Clean (Lot 1) → Sends task to ASC Purchasing Team.

·       Arrange Deep Clean (Lot 2) → For block contract; no purchasing task needed.

·       Cancelled → If not proceeding.

(Next action availability depends on the Clutter Image Rating category.)

Mosaic Update: Hospital Referral process

We have now made changes in Mosaic to streamline discharges into residential or nursing placements.

The process differs for new referrals vs. people already receiving community services prior to admission.

 

 What has changed

1. Additional next actions added to the Hospital Referral workflow step:
• Progress to Conversation 3
• Review to be brought forward

2. Updates to the Follow‑up section of the Hospital Referral form:
• New mandatory question: “What step are you progressing to?”
• Mandatory questions appear only when progressing to Conversation 3
• These questions remain hidden when progressing to Conversation 2
• All other scenarios remain unchanged


Why this change has been made

a) For brand new referrals (people not previously in receipt of services):


• You can now progress directly to Conversation 3 from a Hospital Referral when arranging a residential or nursing placement.
• Additional questions capture essential information that would previously be entered in a Conversation 2 form (which you will be bypassing if choosing this route).
• When progressing to Conversation 3, please select ‘Send Immediately’ in Next actions, so the Hospital Referral remains open until all required information is added.

 

b) For people who previously received community services:

• You can now end the Hospital Referral by selecting ‘Review to be brought forward’.
• The Hospital Team should then complete the existing Support Plan Review.

 


Next steps after completing Conversation 3 or Support Plan Review

Once the work is complete, please assign the Initial (6‑week) Residential/Nursing Review to the relevant team folder.
