Mosaic Update: Hospital Referral process

We have now made changes in Mosaic to streamline discharges into residential or nursing placements.
The process differs for new referrals vs. people already receiving community services prior to admission.
What has changed
1. Additional next actions added to the Hospital Referral workflow step:
• Progress to Conversation 3
• Review to be brought forward
2. Updates to the Follow‑up section of the Hospital Referral form:
• New mandatory question: “What step are you progressing to?”
• Mandatory questions appear only when progressing to Conversation 3
• These questions remain hidden when progressing to Conversation 2
• All other scenarios remain unchanged
Why this change has been made
a) For brand new referrals (people not previously in receipt of services):
• You can now progress directly to Conversation 3 from a Hospital Referral when arranging a residential or nursing placement.
• Additional questions capture essential information that would previously be entered in a Conversation 2 form (which you will be bypassing if choosing this route).
• When progressing to Conversation 3, please select ‘Send Immediately’ in Next actions, so the Hospital Referral remains open until all required information is added.
• You can now end the Hospital Referral by selecting ‘Review to be brought forward’.
• The Hospital Team should then complete the existing Support Plan Review.
Next steps after completing Conversation 3 or Support Plan Review
Once the work is complete, please assign the Initial (6‑week) Residential/Nursing Review to the relevant team folder.
