Case Recording

All ASC practitioners have a responsibility to make high quality case records in line with the Care Act 2014. Good case recording is an integral part of the service we provide to adults and carers. Accurate case recording of up to date information is essential to maintaining high standards of social care practice and service delivery.

As outlined in the Care Act 2014, individuals, carers and families must be involved in the recording process. Their signatures should also be obtained on assessments, statements of need and care plans/action plans. The individual and carer’s views should be accurately recorded. This should be clearly evidenced in both case notes and assessments.

Individuals and carers should be informed about:

  • what information about them will be contained in a case record
  • what information is to be shared with other professionals or agencies
  • why it is being shared.
  • informed about their right to access their case record and how to do it

Equality and valuing diversity

Case recording should reflect the council’s equality and valuing diversity policies and promote anti-discriminatory practices. Case records should indicate any steps taken to facilitate participation of individuals in assessments or contact who have communication difficulties relevant, this also includes providing information and advice in accessible formats.
