What Matters: Three Conversations

What Matters is proudly Camden’s approach to delivering Adult Social Care (ASC) support since 2019.  It is our attempt at placing people at the heart of everything that we do, and it recognises that people are the experts in their own lives.  Through the approach, we support people, carers, families, and communities to focus on what really matters to them and collectively, we find the best solutions to the difficulties they face.

Individuals must be made aware of their indicative budget, how it was calculated, what may be available to meet their eligible needs, the total amount that the council will pay and what amount (if any) the person will pay towards their care and support. Individuals should also be informed (including in writing) of the different options regarding how their budget and care will be managed, including:

  • direct payments
  • managed by the council
  • individual service fund (managed by the provider or third party on the persons behalf).
  • brokerage service

Sign off should occur once:

  • it has been agreed how needs will be met
  • final budget has been discussed and agreed

Agreement of the care and support plan should be signed off by the social worker, individual and persons involved in the care and support planning process, additionally a copy of the care and support plan should be given to the individual and their advocate.
