Information and guidance to Mosaic

Mosaic

Mosaic is the main database for Adult Social Care and for Childrens services. It's a records and finance management system.

A new way of referring to the Sensory Needs Team will be implemented on Mosaic soon.

 
You will still be able to refer to Sensory Needs from your What matters workflow, however there is an additional screening form to complete within the step.
Available in a Conversation 1, 2, 3, Support Plan Updates/ Review, What Matters- Conversation update, and OT Assessment.
  • Go to the forms/ letters icon (where your optional forms are kept) and create a Sensory needs- screening- Deaf & hard of hearing form or a Sensory needs- screening- Blind & partially sighted form. Both forms can be completed if relevant.
  • In Next actions, select Sensory needs- Initial record and send this to the option available- Virtual Contact Sensory
This is replacing the current way of referring to Sensory Needs, which was simply to choose a Next Action of Sensory Needs Assessment and send it to either the Deaf and Hard of Hearing folder or Vision folder.
