All records made about someone who appears to be vulnerable or has additional needs will be factual and where possible, made with informed consent from the resident in question.
Throughout the duration of a services contact with a tenant, case notes are to be used to track communication and updates to the housing or tenant’s circumstance.
Vulnerability information is reviewed every three year via tenancy visits, or sooner if there are significant changes in a tenant's circumstances which comes to the attention of a staff member, such as recovery from a medical condition, bereavement, or changes in household composition.
