Knowing the signs and responding to an early identification of a vulnerability

When a vulnerability is first identified, the responding staff member should add a case note to the tenant’s file which includes key information such as:
Information on the vulnerability and/or relevant events
Details of any risks identified
Preferences and needs
Support offered
Agreed actions or next steps
Any other agencies working with the resident
Any other relevant information e.g. mental capacity, triggers, warning signs
Case notes should be updated with relevant and useful information throughout the duration of support. For more information, see our recording section.
