Ways of identifying a vulnerability

There are multiple ways in which we may come to identify a resident’s vulnerability across Housing and Repairs. This includes: 

  • Self-reporting – a resident might tell us during assessment, home visits, or conversations 

  • Reviewing case history – spotting patterns or concerns in case records. no contact, frequent no access 

  • Reports from others - Concerns raised by neighbours, family members, advocates, or other residents 

  • Direct observation seeing indicators like neglect, clutter, or poor self-care  

  • Information Sharing - Insights shared by other teams, services, agencies, or multidisciplinary meetings
