No single agency or individual can see the complete picture of the life of a resident, but all may have insights and can provide interventions that are crucial to their safety and wellbeing.
Working closely with other services and agencies to deliver a coordinated approach is essential when responding to a tenant’s vulnerability.
All professionals working within a multi-agency framework should:
Share proportionate and relevant information
Take part in joint assessments or multi-disciplinary meetings
Provide an integrated service delivery to the tenant and household
Agree upon designated lead professional coordinating the response and acting as a central point of contact with the tenant or household
Have open communication and space for reflection together
