Disability

At the heart of providing person-centred support for people with disabilities is recognising the whole person, including needs, preferences, and wishes. How we tailor the support provided will vary according to the disability and how this affects them daily.

 

Support at Camden

Adult Social Care: If a resident has an ongoing health condition or complex multiple needs, Adult Social Care can provide longer-term support to help them maintain independence and live well.  

Residents may need extra support if they: 

  • provide unpaid care for a friend or relative 

  • have a hearing or visual impairment that affects day to day living 

  • have a learning disability 

  • have a physical disability 

To contact Camden Adult Social Care and request a discussion about their care and support needs, residents can self-refer via the form: Residents/Self Referral Form 

Involved professionals can contact Camden Adult Social Care on behalf of someone else, by completing this form: Professional Referral Form 

This referral form is for access to Adult Social Care for help with care and support needs and may form part of an assessment under the Care Act 2014. This assessment may include Social Care, Occupational Therapy or Sensory Needs services. It does not cover Mental Health, Learning Disabilities, or Safeguarding.

The Sensory Needs Service for Adults provides specialist team that supports people who live in Camden who are: 

  • certified or registered blind (severely sight-impaired)  

  • partially sighted (sight impaired)  

  • have a non-correctable sight loss in both eyes 

  • are Deaf or hard of hearing 

  • have both a significant hearing and sight loss

Camden Learning Disabilities Service (Clds) - provide a range of services for people with learning disabilities, as well as autism. There is an online form tenants can complete or get help, or a referral form which can be completed and sent on their behalf by a professional and sent to [email protected] or [email protected]. 

Extra care housing supports people who have social care needs to live independently in their own home, whilst maintaining their privacy within an accommodation-based setting. The scheme provides high quality care and support to adults who have been assessed as having needs that are eligible for social care funding, including those arising from varying degrees of physical frailty, learning disability, mental ill health and cognitive impairment, including dementia, or a combination of these. Camden’s Careline service can support tenants by providing assistive technology. Assistive technology describes a wide range of equipment or devices that can help people to live independently by supporting or assisting them in daily living tasks. 

Accessible venues: We have multiple accessible venues which we can use across the borough. This includes some TRA halls, libraries, and Crowndale Conference Centre. Some of these venues have parking, on-site and to arrange we should contact the venue.  Where we can services should provide information sheet on how to find the venue (including photograph, map, address) to anyone who needs it.
