By this point, you will have done the exploration work, identified strengths and needs, and these will form the basis of your assessment. You'll be writing that up using the template on Mosaic.

You'll also have worked together to come up with some goals ("where do we want to be as a family"), and these form the basis of the family's plan. The plan should help a family know what steps and actions to take to achieve their goals. 

This tool will help you and the family generate ideas for what action needs to happen to achieve goals (remember a 'ladder of goals' - if a goal feels too big, too small, or not relevant, then its unlikely you'll be able to make a plan that's going to work either)
