Social Prescribing

In Camden, the social prescribing service is delivered by a partnership of organisations (Age UK Camden, Voluntary Action Camden, and the Council’s Wish Plus referral hub)

In Camden, the social prescribing service is delivered by a partnership of organisations (Age UK Camden, Voluntary Action Camden, and the Council’s Wish Plus referral hub) that provides a single point of access to assess, triage, advise and support residents that are referred.

The single point of access and triage process ensures that all residents are connected to the right support without delay.

The partnership comprises:

  • Community Links team – typically the first point of access for the service. The team support residents with less complex health needs by referring them to the right community-based services and activities, or a range of social prescribing support
  • Care Navigators team – for more complex needs, they provide personalised support and assessment to help identify needs and support navigation through the local care system
  • WISH Plus team – for resident that require specific information and advice on keeping warm, income and safety around the home as well as some health and wellbeing services.
