The service operates on an ‘open’ social prescribing model that enables referrals to be received from all health and care services, as well as referrals from residents themselves.  This ensures that Camden residents aged 18 plus who are registered with a GP in Camden, may be offered the support available to improve their health and wellbeing, whichever route the person arrives into the service.
