Quick guidance regarding Shared Lives Schemes for practitioners

Shared Lives

Shared Lives Camden is a service where adults with care needs are given the opportunity to live in the homes of people who will support them.  People with care needs will share a home with a carer and in some instances the carer and their family and will be supported to lead an independent, healthy life as part of the community.  These arrangements can be short stays and something more long-term.

Staff including social workers will know the person who they think would be suitable for the shared lives scheme. 

However, it is good practice that before contacting the shared lives team, that you firstly consider whether the person meets the criteria.  If they do, you should talk to the person you wish to refer and explain what the service is and what can be offered.

Once this has been completed your next steps are to:

  • Speak to the team to discuss the persons needs, this will give a good indication if the person will be suitable for shared lives.
  • Complete mosaic referral which you will have the option for in conversation 3 in the adult workflow, this is a deliberately short form.
  • Prepare the latest review document, Shared Lives staff will contact you to find out more information and complete the next forms.

Shared lives staff will be busy in the background but will keep referrers updated and will make contact when they need further information, however if as a referrer you require additional information they can always be contacted at any point during the process.
