Referral process for Extra Care housing
Eligibility Criteria
Tenants eligible for the scheme will be residents who:
• are deemed to have Ordinary Residence in Camden as defined by the Care Act 2014
• have been assessed as having an eligible support need under the Care Act;
• have capacity to independently manage a tenancy agreement or have someone legally appointed to sign the tenancy on their behalf Further information and guidance on tenancies can be found on the tenancies page.
In line with Camden’s housing allocation scheme, a resident’s savings and assets will be considered when allocating Extra Care. If the person is not eligible for Housing Benefit, they will be liable for the full rental costs. Self-funders and people who own their own home are eligible to access Extra Care but must be Camden residents.
Residents will be expected to fulfil the responsibilities of their tenancy agreement, which includes paying rent and bills.
Residents with leave to remain are eligible for an Extra Care tenancy for the duration of their right to remain in the country. Residents with no recourse to public funds are not eligible for Extra Care.
Referral Process
New processes are in place for Extra Care applications.. The following referral process applies to all Extra Care Schemes with the exception of Charlie Ratchford Court. A separate guide to CRC referrals is available in the Charlie Ratchford Court section (see to the left of this page).
- Practitioner to ensure accurate care and support plan is in place for discussion with their line manager, this should include all relevant documentation and consideration of the resident’s eligibility for Extra Care as outlined above. Note: Comprehensive, recent information is vital to ensure that the resident is accepted to the right scheme to meet their needs and to minimise the risk of future placement breakdown.
- The practitioner should also provide a proportionate summary of the situation and rationale for the application for Extra Care in Mosaic case notes or within the assessment documentation if the practitioner has no access to Mosaic.
- Once this has been completed, the presenting practitioner/clinician is to email the name of the client, mosaic ID number and supporting documents to [email protected] who will forward the details to relevant managers on the panel for consideration of eligibility.
- Practitioner to present the case to QAP where a decision may be made immediately or panel members will advise on next steps/ additional information required. Practitioners are to contact Team Clerks to arrange their presentations. Please note, cases cannot be presented on the same day as submission (unless exceptional or emergency circumstances apply, and these should be discussed with line managers as soon as they arise).
- Following approval at QAP, a call will be scheduled with the Extra Care Development Officer ([email protected]) to discuss available voids and which scheme/s would be best placed to meet the needs of the resident
NOTE – A scheme may decline a referral after a resident has been approved by QAP for Extra Care, therefore practitioners are advised to be transparent with residents throughout the process and to manage their expectations accordingly. - Practitioner to send a task via Mosaic to the Purchasing Team, confirming the preferred Extra Care scheme/s
- The Placement Officer will make the referral to the relevant Extra Care Scheme Manager and arrange for an assessment to be carried out, usually within one week
Where the resident is accepted:
• Extra Care Development Officer provides relevant information and advice to prepare for tenancy sign up
• Preparations begin for move-in
Where the resident is declined:
• Feedback is provided by the Extra Care Scheme Manager for review by the Practitioner and Extra Care Development Officer
• Further information may need to be provided at this stage
• Resident may be referred to an alternative scheme
Where the resident is declined by all available Extra Care schemes:
• Practitioner and their line manager will be informed
• Alternative options must be explored e.g. care at home / residential care etc.
