Prison Release Guidance
The Prison Release Practice Guidance provides a structured framework for how Housing Services and Adult Social Care work together to support residents being released from prison who have care and support needs.
The guidance outlines clear processes for early identification of vulnerability, coordinated risk assessment, information sharing, and joint planning to ensure no individual is discharged into unsafe or unsupported circumstances. It sets out expectations for ASC attendance at monthly multi‑agency resettlement meetings, procedures for managing unplanned or emergency releases, and defined communication pathways between Housing, ASC Neighbourhood Teams and Mental Health Teams.
The guidance emphasises shared accountability, timely escalation of concerns, and a principle that no one should “fall through the gaps.”
