Housing Matters
Referrals for the Mental Health Pathway are through the Camden and Islington Mental Health Service and details should be recorded on Mosaic. The funding is agreed at fortnightly mental health funding panel meetings. The co-ordination of placements for named projects is managed by Camden's Mental Health Referral Coordinator.
Practitioners will need to:
- complete a PURF (Pathway Universal referral form) and signed privacy notice for individuals moving to and within the Pathway. The PURF form should be clear on recommendation for Pathway, need for a move, support needs and level of support the client requires.
- inform the Pathways Move on Team of the date and room/flat number as soon as possible when an individual moves in or out of the Pathway
If an individual is moving to independent living from the Pathway, the project is responsible for completing PITAF (Pathways Independent Tenancy Assessment form). Supporting documents should be submitted with the form and if the individual is bidding for social housing their current CPA will need to be included.
The care team will discuss accommodation for individuals who require residential care, extra care sheltered or any other alternative funded placement.
Sideway moves to the Adult Hostels Pathway can be made directly via SPOCC (Supporting People Oxford Computing Consultancy) from Pathways projects.
