Best practice to ensure that patients are safely discharged from hospital

Hospital discharge

Hospital discharge refers to when a patient is ready to leave hospital and return to their normal place of residence. This section provides best practice to ensure a person is discharged safely, including: discharge pathway, reablement flats, safeguarding raised in hospital, discharge alert protocol, and support for carers that are identified as part of the discharge process. 

Urgent community response is a multi-disciplinary service which is designed to provide urgent care for Camden residents who are above the age of 18 and are at risk of hospital admission due to physical health deterioration, reduced mobility and/or unexpected carer breakdown. There is a social worker from ASC working alongside the Camden UCR team. The SW typically support patients who are having nursing and/or therapy input at home from UCR team and who would benefit from urgent social care assessment to avoid hospital admission. These patients would initially have Carelink involvement before being referred to the UCR SW.
