Best practice to ensure that patients are safely discharged from hospital

Hospital discharge

Hospital discharge refers to when a patient is ready to leave hospital and return to their normal place of residence. This section provides best practice to ensure a person is discharged safely, including: discharge pathway, reablement flats, safeguarding raised in hospital, discharge alert protocol, and support for carers that are identified as part of the discharge process. 

Reablement flats are temporary accommodation offered to individuals to support them to improve, develop and regain confidence. The flat is available only if reablement support cannot be offered in the individual's own home. The individual's care and support are reviewed during their stay and the maximum stay period is 6 weeks.

Practitioners are required to complete a referral form for the reablement flat. The form should be sent to [email protected] (single point of access). Community referrals for Henderson Court should be completed by social care or health-based community practitioners and the form should be sent to [email protected]. Referrals should consider:

  • Why the person can’t receive reablement in their own home?
  • Does the person have a permanent residence of their own and is the plan for them to go home after reablement?
  • Does the person have ‘Ordinary residence’ rights within Camden?
  • If the person is homeless, awaiting a housing transfer or has an uncertain onward destination consideration should be taken on what are the housing solutions in place for onward destination

The practitioner should write up an initial support plan and send to Reablement provider before admission. Therapists should arrange any equipment required and inform Reablement provider about delivery.
