Assistive Technology (Careline Telecare)

Everything you need to know about the Careline service and how to utilise the amazing potential of assistive technology to promote independence and deliver effective prevention.

To make changes or stop the Careline service, the person should telephone Careline. If this is not possible then a practitioner, carer or family member may contact Careline instead.

Practitioners should notify Careline if they become aware that the person has been admitted to hospital, access to their home has changed, they are moving or have died. Practitioners should phone or email Careline.

The Careline provision in MOSAIC will be updated when any change /end in service is made that is expected to be permanent or indefinite.

Systems:

  • Person's record will be updated or closed in Jontek
  • Provision will be updated/cancelled by Careline in Mosaic

 

Guidance Notes:

Scenarios might include:

  • Change in phone line/keys /key safes/contacts
  • Moving home (but no change in type of tenure)
  • Changes in health /care arrangements /needs /no longer want Careline
  • Moving into supported living /residential /nursing care
  • Death

It has been identified that practitioners need a checklist of tasks and providers to contact when a person is moving home, to a different care setting, is going into hospital or has died. Careline should be contacted regarding changes in circumstances – practitioners will know that it is relevant because the individual will have an AT provision recorded on their care record.
